Walk though key features, install myFHR, complete the registration, and view frequently asked questions.
Key Features
myFHR allows you to:
- Go prepared to your next doctor visit with mobile access to all of your health records.
- Prevent errors by helping your care providers act on consistent knowledge of your medications, allergies, and other critical information.
- Securely store images and scans of health documents.
- Add over-the-counter medications and track symptoms like pain or mood.
- Be alerted to important health events—abnormal lab results, overdue vaccinations, and readings from connected smartwatches and devices.
- Share access with family, friends, caregivers, and your health plan.
System Requirements
For Apple devices, myFHR is supported on iPhone, iPad, Apple Watch and iPod touch running iOS 16.4 and higher.
For Android devices, myFHR requires Android version 10.0 or higher.
The web version of myFHR is supported in most modern web browsers (this does not include Internet Explorer), but works best in Google Chrome.
Only Apple and Android devices support sensor data collection, and the available data varies by device.
Downloading the myFHR app
The myFHR app is currently available for iOS and Android. You can also find it by searching for "myFHR" or "CareEvolution" in the Apple App Store or the Google Play Store.
Completing Registration
After downloading the myFHR app, you'll need to register your account. There are several ways to begin:
If your health plan is with Anthem, you must select the Anthem option during registration to import your Anthem health data into myFHR. You will not be able to import your Anthem health data if you use one of the other registration options.
- Touch "Join" to open the registration screen.
- Touch "Anthem" to open the Anthem patient portal. Log in to the portal using your anthem.com member account credentials, and follow the instructions to allow access to your account. The registration screen opens.
If you used the "Anthem" button when registering for myFHR, you will also need to use the "Anthem" button when logging in to myFHR (using your anthem.com member account credentials).
- Touch either "Facebook" or "Google" to open their login screens. After logging in, the registration screen opens.

Complete the resulting screen to register your account.

A popup then displays asking if you will allow myFHR to send you notifications. The notifications may include alerts, sounds, and icon badges. Select "Allow" to enable notifications. The notifications can be configured in your phone's settings.
After you connect to Apple Health (iOS device users), myFHR will send you an email to verify your email address. If you don't receive the email, touch "Please verify your email address" to resend it.
If you need to resend the verification email, keep in mind that only the last verification email sent is valid. Make sure to select the new, unread message in your inbox. The link in the verification email will expire within an hour. Make sure to click the second link in the email, and not the link that is your email address.
Adding a Photo to Your Profile
To add a photo to your profile, touch the placeholder image to the left of your name.

Using the resulting screen, you can take a photo or choose a photo stored on your phone. The photo taken or chosen displays.
